If you are getting insurance through your place of business, it’s up to them what they want to cover or not. If you are getting it independently, then you need to talk to them about what you want to pay for and what you don’t want to pay for.
If you are referring to employer’s insurance, the covered items will be different depending on what the company negotiated.
If you are referring to an independent insurance policy, everything is negotiable and dependent on what you want to pay each month.

I know Kaiser and Aetna both have plans that cover infertility, but alot of insurances only cover certain procedures and sometimes only up to 50%. It can still be quite expensive. I have spent an average of $800.00 per cycle on clomid and insemination, so you still should be prepared to spend some money, however that 50% is helpful. Most insurances do not cover ivf.
